Job Summary

The Outreach Health Education Coordinator will serve as part of the University of Kentucky Bluegrass Care Clinic (BCC) multidisciplinary team to identify out-of-care persons living with HIV (PLWH) and assist them with reestablishing care through linkage and re-engagement services.

This position will provide counseling and education to clients to improve health literacy, increase retention in medical care, improve treatment adherence, and help clients achieve better health outcomes. This position will support testing initiatives and outreach efforts in the identified Lexington/Fayette Linkage Region as well.

The Outreach Health Educator Coordinator will coordinate services with CDC prevention specialists and other community partners and link PLWH to appropriate services, contributing to the Bluegrass Care Clinic’s mission of providing quality services to over 1,800 HIV positive patients living in Central and Eastern Kentucky.



Skills / Knowledge / Abilities

MS Windows, Excel, Access (or other database experience), Outlook. Written/oral communication, planning/organizing, continuous improvement, medical knowledge, teaching skill, collaboration, attention to detail, basic statistics, quantitative analysis, and computer skills.

Master’s degree in Health Education, Social Work, Social Services, Public Health, Epidemiology, Criminal Justice, or Human Development
